Purpose
A community & strength based planning process that promotes family preservation & independence
- To qualify for ICCW, children, youth, or young adults (ages 0-21) must have a Serious Mental Illness (SMI), Serious Emotional Disturbance (SED), and/or an Intellectual / Developmental Disability with complex behavioral needs and an identified home and community.
- Additionally, at least two of the following criteria must be met:
- Involvement in two or more systems (e.g., mental health, criminal justice, welfare, special education).
- Risk of or currently in an out-of-home placement, recent discharge from state hospital/CCI, recent inpatient psychiatric stay, or release from a non-medical facility in the last year.
- Lack of support system.
- Requirement for higher intensity services than prior case management.
- Unstable complex medical needs.
- Frequent school suspensions or expulsions.
- Significant distress following a traumatic event.
